RELEASE
I understand that this dog is a rescue, and I accept the animal as is and assume all risks of its ownership, including the risk of injury or damage caused by the animal (such as animal biting someone), as well as sickness or disease, or death of this animal, or my other animals. On behalf of myself, my heirs, personal representatives and assigns, I hereby release, discharge, indemnify and hold harmless Lisa Doyle, any and all claims, causes of action and demands of any nature, whether known or unknown, arising out of or in connection with my adoption. By signing, I am attesting to the truthfulness of my answers. Falsification of any information, or failure to spay/neuter, shots, etc.. the dog by the above date, providing Lisa Doyle, with proof, will be grounds for Lisa Doyle, to confiscate the said dog. I aggree not to give this dog away, or dispose of it in any manner, without written authorization, from Lisa Doyle. Applicant must be 18 years of age or older. Lisa Doyle reserves the right to refuse any applicant.
